Writing a Nursing Care Plan

Nursing plan has a history of more than 70 years. It was first developed in the 1930s as a method to convey patient care instructions to nursing aides and other members of the health-care team. Today, hospitals are including nursing care plans in the electronic patient record, which has resulted in the increased use of standardised care plans.

While most hospitals use either printed or electronic nursing care plans and provide care plan templates covering a wide variety of nursing diagnoses, a patient may occasionally present with a problem where an appropriate template is not available.

This whitepaper provides several guidelines to generate several parts of nursing stuff and how to combine them into an individualised nursing care plan.

These topics are: Assessment, Nursing diagnosis, Planning, Nursing interventions and Evaluation.
