This three-page fast fact sheet provides an outline of the process necessary in creating a care plan; as well as indicating some of the different international methods of documenting and writing care plans. Although originally designed to complement the 'Care Planning' PowerPoint Presentation, this fast fact sheet is equally useful on its own; as a Hints & Tips sheet for the necessary health professionals.

Aurora Cyganik-Barker, RN, BN is a degree-educated Registered Nurse, who graduated in 2007 with distinction from the University of Calgary. She is currently living and working in Western Canada. Aurora worked as an undergraduate nurse for 2 years while completing her nursing degree in the geriatric assessment unit at Medicine Hat Regional Hospital under geriatricians Dr. Sahiholnasab and Dr. Villaneueva. Her experience has a wide range including working in emergency and the cardiac/ICU step-down unit at Medicine Hat Regional Hospital for 2 years. She also worked during this time at Bow Island Hospital, a rural hospital, where she divided her time between a 10 bed acute wing, a 20 bed long term care wing and a busy outpatient/emergency department. She very recently moved into a Primary Care role in a single physician’s office, concentrating on chronic disease management and prevention. Although chronic disease management is her latest passion, she also has a special love for geriatrics, and has the opportunity to work with many older adults in her current position. She is also a certified massage therapist, Reiki master and has a special interest in alternative and complementary therapies.
